Toelichting: Het voorwerp van de opdracht betreft de levering van Lenalidomide 2.5 mg, 5 mg, 7.5 mg, 10 mg, 15 mg, 20 mg, 25 mg capsules aan de ziekenhuisapotheek Het betreffende geneesmiddel dient in elk geval geregistreerd/vergund te zijn voor het in de handel brengen in België conform de betreffende wetgeving en bovendien vergoedbaar te zijn conform de regelgeving van het RIZIV. Gelet het huidige patiëntenbestand behandeld met Lenalidomide, dient de inschrijver aan te tonen dat er minimaal vergoedbaarheid conform de regelgeving van het RIZIV beschikbaar is m.b.t. alle onderstaande paragraafnummers volgens Hoofdstuk IV van de lijst vergoedbare farmaceutische specialiteiten (zoals beschreven in K.B. van 01.02.2018), en dit gedurende de volledige looptijd van de overeenkomst. De vergoedbaarheid binnen deze paragrafen is een minimale vereiste (cf. III: Minimale eisen).
