Description

Postępowanie prowadzone jest w trybie przetargu nieograniczonego na podstawie ustawy z dnia 11.09.2019r. Prawo zamówień publicznych. Szacunkowa wartość zamówienia przekracza kwotę określoną w obwieszczeniu Prezesa Urzędu Zamówień Publicznych wydanym na podstawie art. 3 ust. 2 p.z.p. Zamawiający przewiduje zastosowanie tzw. procedury odwróconej, o której mowa w art. 139 ust. 1 ustawy PZP, tj. Zamawiający najpierw dokona badania i oceny ofert, a następnie dokona kwalifikacji podmiotowej Wykonawcy, którego oferta została najwyżej oceniona, w zakresie braku podstaw wykluczenia oraz spełniania warunków udziału w postępowaniu. Przedmiotem zamówienia jest „Dostawa zestawów do ręcznej i automatycznej dializy otrzewnowej CADO ADO”. Przedmiot zamówienia obejmuje 5 niepodzielnych części, dla których Zamawiający dopuszcza możliwość składania ofert częściowych, z zastrzeżeniem, iż oferta w każdej z nich winna być pełna i powinna spełniać szczegółowe wymagania określone w formularzu cenowym. Część 1 - Zestawy ADO z podwyższonym pakietem bezpieczeństwa do kontynuacji leczenia; Część 2 - Zestawy CADO z podwyższonym pakietem bezpieczeństwa do kontynuacji leczenia; Część 3 - System CADO - o jednym połączeniu typu „TWIN BAG” do kontynuacji leczenia; Część 4 - System ADO do kontynuacji leczenia; Część 5 - System ADO
