Description

1. Przedmiotem zamówienia jest dostawa sond FFR, cewników IVUS oraz prowadników na potrzeby Pracowni Hemodynamiki Samodzielnego Publicznego Zakładu Opieki Zdrowotnej Wojewódzkiego Szpitala Zespolonego im. Jędrzeja Śniadeckiego Białymstoku. W Załączniku nr 2 do SWZ - Formularz Szczegółowy Oferty określono ilości i wymogi jakie musi spełniać oferowany przedmiot zamówienia. 2. Opis przedmiotu zamówienia oraz warunki realizacji zamówienia zawarte są w załącznikach: a) Formularz Szczegółowy Oferty (opis przedmiotu zamówienia dla poszczególnych pakietów (części) - Załącznik nr 2 do SWZ; b) Projekt umowy (warunki realizacji zamówienia) Załącznik nr 7.1 do SWZ (Pakiet nr 1); c) Projekt umowy (warunki realizacji zamówienia) Załącznik nr 7.2 do SWZ (Pakiet nr 2). 1.
