1. Przedmiotem zamówienia jest świadczenie na rzecz Szpitala Wojewódzkiego w Bielsku-Białej, zlokalizowanego przy Al. Armii Krajowej 101, 43-316 Bielsko-Biała, świadczenie usług serwisu nad Zintegrowanym Systemem Informatycznym InfoMedica/AMMS firmy Asseco Poland S.A., i infrastrukturą sieciową eksploatowanymi w jednostkach organizacyjnych Szpitala Wojewódzkiego w Bielsku-Białej, na warunkach określonych w projektowanych postanowieniach umowy stanowiących Załączniku Nr 5 do swz. 2. Wykaz elementów objętych świadczeniem usług serwisu, o której mowa w ust. 1, zawiera Załącznik Nr 2 do swz - Formularz cenowy. 3. Każdy moduł wymieniony w Załącznik Nr 2 do swz posiada ilość licencji - open (bez limitu użytkowników/ nieograniczona ilość stacji roboczych). 4. Wykonanie przedmiotu zamówienia będzie się odbywać na koszt i ryzyko Wykonawcy.
