Przedmiotem zamówienia niniejszego postępowania przetargowego są: dostawy materiałów do ciągłych terapii nerkozastępczych i plazmaferezy wraz z dzierżawą czterech aparatów, zwanych dalej towarem, zgodnie z rodzajem asortymentu, spełniających opis i parametry graniczne opisane w załączniku nr 1a do SWZ, szczegółowym opisem i ilościami określonymi w Formularzu asortymentowo-cenowym stanowiącym załącznik nr 2 do SWZ oraz zgodnie z Formularzem Oferty stanowiącym załącznik nr 1 do SWZ.
