Description

Przedmiotem zamówienia jest dostawa aparatury medycznej - 5 zadań. Postępowanie o udzielenie zamówienia publicznego prowadzone jest w związku z realizacją projektu pn. „Uniwersyteckie Centrum Ambulatoryjnej Opieki Specjalistycznej - optymalizacja piramidy świadczeń w woj. lubelskim poprzez rozbudowę i wyposażenie Poradni Specjalistycznych USK Nr 4 w Lublinie”, nr FENX.06.01-IP.03-0051/25, realizowanego na podstawie Umowy nr FENX.06.01-IP.03-0051/25-00/53/2026/84 z dnia 05.03.2026 r., w ramach programu Fundusze Europejskie na Infrastrukturę, Klimat, Środowisko 2021-2027, Priorytet FENX.06 Zdrowie, Działanie FENX.06.01 System ochrony zdrowia. Szczegółowe wymagania w stosunku do w/w asortymentu zamówienia i zakres zamówienia zawiera Załącznik nr 1.1 - 1.5 (odrębny plik xls) oraz Załącznik nr 1.1a - 1.5a (odrębny plik word) do niniejszej SWZ. Realizacja przedmiotu umowy nastąpi w ciągu 6 tygodni od dnia zawarcia umowy, jednak nie później niż do 14.12.2026 r. Dostawa aparatów do badań ginekologiczno - położniczych - 2 kpl.; Dostawa aparatu USG do badań ginekologiczno - onkologicznych - 1 kpl.; Dostawa kolposkopu ginekologicznego - 1 kpl.; Dostawa cystoskopów. CZĘŚĆ A: Cystoskop - 10 kpl. W skład 10 kompletów wchodzi: Cystoskop typ I - 8 szt. i Cystoskop typ II - 2 szt. Łączne wyposażenie dla 10 kompletów: kosz do sterylizacji cystoskopu - 5 szt., wideoprocesor z monitorem - 1 szt., wózek do wideoprocesora - 1 szt., jednostka sterująca - 1 szt., tor wizyjny - 1 szt. CZĘŚĆ B: Cystoskop - 2 kpl. W skład 2 kompletów wchodzi: Cystoskop typ III - 1 kpl. i Cystoskop typ IV - 1 kpl.; Dostawa aparatu do urodynamiki - 1 kpl.
