Description

1. Przedmiotem zamówienia jest dostawa sprzętu medycznego o parametrach technicznych określonych w Załączniku numer 4 do SWZ, zwanym dalej „sprzętem”, wraz z instalacją, przeszkoleniem pracowników Zamawiającego oraz pracami adaptacyjnymi, Zamawiający wymaga aby prace adaptacyjne były zrealizowane zgodnie z wymaganiami określonymi w załączniku nr 11 (Dokumentacja techniczna) oraz w załączniku nr 12 (Ochrona radiologiczna). 2. Dokument „Projekt Techniczny, Instalacje elektryczne i teletechniczne” zostanie udostępniony Wykonawcom w formie elektronicznej za pośrednictwem Platformy prowadzonego postępowania po uprzednim zawarciu umowy o zachowaniu poufności stanowiącej załącznik nr 13 do SWZ. Wykonawca zwróci się do Zamawiającego za pośrednictwem Platformy prowadzonego o udostępnienie niniejszych załączników i podpisanie umowy o zachowaniu poufności. 3. Przedmiot niniejszego zamówienia: Aparat PET-TK (1 szt.) o parametrach technicznych określonych w Załączniku numer 4 do SWZ; 4. Wymieniony wyżej sprzęt musi spełniać warunki graniczne określone odpowiednio w Załączniku numer 4 do SWZ. Niespełnienie chociażby jednego z warunków granicznych określonych odpowiednio w Załączniku numer 4 do SWZ spowoduje odrzucenie oferty.
