Description

3.1. Przedmiotem zamówienia jest dostawa materiału szewnego, staplerów i siatek do Centralnego Bloku Operacyjnego w szpitalu w Łęcznej. Przedmiot zamówienia został szczegółowo opisany i wyszczególniony w załączniku nr 2 do SWZ. Przedmiot zamówienia jest podzielony na 4 zestawy : Opis zestawu Oznaczenie wg Wspólnego Słownika Zamówień: CPV: zestaw nr 1- materiał szewny;33141121-4 Szwy chirurgiczne zestaw nr 2- elektryczna rękojeść staplera;33141100-1 -Opatrunki, zaciski, szwy, podwiązki; Zestaw nr 3- Siatka polipropylenowa;33141121-4 Szwy chirurgiczne Zestaw nr 4- Siatka kompozytowa;33141121-4 Szwy chirurgiczne 3.2. Przedmiot zamówienia musi być oznaczony zgodnie z obowiązującymi przepisami prawa oraz dopuszczony do obrotu i używania na terenie Polski, posiadać niezbędne atesty i certyfikaty, pozwolenia, jeżeli są wymagane odpowiednimi przepisami. 3.3.Dopuszcza się składanie ofert częściowych na poszczególne zestawy. W ramach zestawu należy zaoferować wszystkie wymienione pozycje. Nie wypełnienie którejkolwiek pozycji w ramach części będzie skutkowało odrzuceniem oferty. 3.4.Podane w załącznikach parametry techniczne należy traktować, jako wzór spełniający minimalne wymagania. Proponowane przez Wykonawców elementy nie mogą posiadać parametrów gorszych od podanych, jako wzór. 3.5.Zamawiający dopuszcza zaoferowanie przedmiotu zamówienia w opakowaniach zawierających inną ilość przedmiotu zamówienia niż sugerowana w tabelach formularzy cenowych, z jednoczesnym przeliczeniem ilości. 3.6. Przedmiot zamówienia ma być nowy, dopuszczony do obrotu na podstawie obowiązujących przepisów prawa i odpowiadać wszelkim wymaganiom określonym przepisami prawa, w szczególności ustawą z dnia 7 kwietnia 2022 roku o wyrobach medycznych (Dz. U. 2024 poz. 1620) oraz wolny od jakichkolwiek wad fizycznych lub prawnych oraz posiadać w dniu dostawy termin ważności nie krótszy niż 12 miesięcy. zestaw nr 1- materiał szewny; zestaw nr 2- elektryczna rękojeść staplera; Zestaw nr 3- Siatka polipropylenowa; Zestaw nr 4- Siatka kompozytowa
