Description

1. Przedmiotem zamówienia jest: Dostawa produktów leczniczych i programów lekowych 2. Przedmiot zamówienia został podzielony na 5 części/ zadań. 3. Szczegółowy zakres zamówienia został określony w Załączniku Nr 1 do SWZ - Formularz asortymentowo cenowy. Pozostałe warunki dotyczące realizacji zamówienia zostały określone we wzorze umowy, stanowiącym Załącznik Nr 5 do SWZ. 4. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części/ pakiety ustalone w Załączniku Nr 1 do SWZ. Wykonawca może złożyć ofertę na dowolne pakiety lub na cały przedmiot zamówienia. 5. Wszystkie wymagania określone w dokumentach wskazanych powyżej stanowią wymagania minimalne, a ich spełnienie jest obligatoryjne. Niespełnienie ww. wymagań minimalnych będzie skutkować odrzuceniem oferty jako niezgodnej z warunkami zamówienia na podstawie art. 226 ust. 1 pkt 5 ustawy Pzp. 6.
