5.1. Przedmiotem zamówienia jest dostawa preparatów do żywienia klinicznego przez okres 12 miesięcy 5.2. Wspólny słownik zamówień: CPV: 33690000-3 Różne produkty lecznicze 33692200-9 Produkty do żywienia pozajelitowego 5.3. Szczegółowy opis przedmiotu zamówienia, opis wymagań Zamawiającego w zakresie realizacji i odbioru określają: 5.3.1. Opis przedmiotu zamówienia - formularz asortymentowo - cenowy - załącznik nr 1 do SWZ z podziałem na 4 części (pakiety) Pakiet nr 1 - Żywienie pozajelitowe Pakiet nr 2 - Żywienie pozajelitowe i dojelitowe Pakiet nr 3 - Żywienie dojelitowe Pakiet nr 4 - Żywienie kliniczne 5.3.2. Projektowane postanowienia umowy - załącznik nr 2 do SWZ. 5.4. Formularz asortymentowo - cenowy stanowi treść oferty i nie podlega uzupełnieniu. Niezłożenie formularza asortymentowo - cenowego wywiedzie negatywne skutki dla Wykonawcy w postaci odrzucenia oferty na podstawie art. 226 ust. 1 pkt 5 ustawy Pzp. 5.5. Opis przedmiotu zamówienia oraz projektowane postanowienia umowy należy odczytywać wraz z ewentualnymi zmianami treści SWZ, będącymi np. wynikiem udzielonych odpowiedzi na zapytania Wykonawców.
