Przedmiotem zamówienia jest: zakup produktów farmaceutycznych. Opis przedmiotu zamówienia znajduje się w załączniku nr 2 do SWZ (formularz asortymentowo-cenowy). Zamawiający dopuszcza składanie ofert częściowych w podziale na 40 części zamówienia /pakiety / zadania. Podział na części określa załącznik nr 2 do SWZ. Zamawiający nie dopuszcza składania ofert wariantowych. Szczegółowe uwarunkowania realizacji dostaw, w tym dostęp do budynków Zamawiającego, określa projekt umowy stanowiący załącznik nr 4 do SWZ. Wymagany termin realizacji dostaw częściowych: 3 dni robocze od złożenia zamówienia. Minimalny termin ważności asortymentu: 12 miesięcy od daty dostawy. Zamawiający zobowiązany będzie do wykorzystania minimum 75% wartości brutto udzielonego zamówienia. Wykorzystanie zależne będzie od liczby pacjentów zgłaszających się do Zamawiającego oraz od umów (kontraktów) z Narodowym Funduszem Zdrowia. Poszczególne części zamówienia realizowane będą niezależnie od siebie.
