1. Przedmiotem zamówienia jest: zakup Nivolumabu (Nivolumabum). 2. Opis przedmiotu zamówienia znajduje się w załączniku nr 2 do SWZ (formularz asortymentowo-cenowy) - Nivolumabu (Nivolumabum) koncentrat do sporządzania roztworu do infuzji 3. Kod określający zamówienie według Wspólnego Słownika Zamówień CPV: 33600000-6 Produkty farmaceutyczne, 33652100-6 Środki przeciwnowotworowe. 4. Miejsce realizacji zamówienia: ul. Grenadierów 51/59, 04-073 Warszawa. 5. Dostawy sukcesywne przedmiotu zamówienia mają być realizowane do Apteki Szpitalnej mieszczącej się w Pawilonie III Szpitala na parterze i do magazynu znajdującego się w podziemiach Szpitala, przy czym każdorazowo Zamawiający wskaże, jeśli miejscem dostawy będzie magazyn. 6. Odbiór przedmiotu zamówienia (asortymentu, produktu farmaceutycznego) odbywać będzie się w pomieszczeniach dostępnych dla pracowników Wykonawcy (dostawców, kurierów).
