Przedmiotem zamówienia są sukcesywne dostawy produktów leczniczych w ramach programu lekowego w leczeniu przewlekłego WZW typu C do Apteki Szpitalnej Zespołu Opieki Zdrowotnej w Bolesławcu. Szczegółowy opis przedmiotu zamówienia zawiera Formularz cenowy - Załącznik nr 2 do SWZ. Każdy Wykonawca może złożyć tylko jedną ofertę na całość zamówienia w formie pisemnej pod rygorem nieważności. W przypadku złożenia przez Wykonawcę więcej niż jednej oferty, wszystkie oferty tego Wykonawcy zostaną odrzucone przez Zamawiającego. Zamawiający zastrzega, że oferowany przedmiot zamówienia musi należeć do grupy produktów leczniczych w rozumieniu ustawy Prawo farmaceutyczne z dnia 6 września 2001 r. Przedmiot zamówienia obejmuje: poz. 1. sofosbuwir/ welpataswir, poz. 2. sofosbuwir/ welpataswir/ woksylaprewir.
