Description

1. Przedmiotem zamówienia jest sukcesywna dostawa PRODUKTÓW FARMACEUTYCZNYCH/LEKÓW, OBJĘTYCH PROGRAMEM LEKOWYM. 2. Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach opisanych we wzorze umowy stanowiącym Załącznik nr 6 do SWZ. 3. Z uwagi na szacunkowe określenie ilości przedmiotu zamówienia, może ulec ona zmniejszeniu, w zależności od potrzeb diagnostycznych pacjentów Zamawiającego, maksymalnie o 10%, zgodnie z zapisami § 1 wzoru umowy. 4. Wykonawca na wszystkie zaoferowane produkty farmaceutyczne/leki, objęte programem lekowym musi posiadać dokumenty prawnie wymagane, na podstawie których zaoferowane produkty farmaceutyczne/leki, objęte programem lekowym mogą być wprowadzone do obrotu, na rynek i do stosowania w placówkach ochrony zdrowia w Polsce. 5. Miejscem dostawy zamówienia jest siedziba Zamawiającego przy ul. Powstania Styczniowego 9b, 81-519 Gdynia - Apteka szpitalna. Wykonawca wnosi zamówione produkty farmaceutyczne/leki, objęte programem lekowym do wskazanych miejsc w siedzibie Zamawiającego. L.p.
