Description

”Dostawa produktów leczniczych”. Szczegółowy opis przedmiotu zamówienia zawiera opis przedmiotu zamówienia tj. załącznik nr 2. do SWZ. Podane w formularzu asortymentowo - cenowym ilości dotyczą okresu 6 miesięcy (na taki okres zostanie zawarta umowa), ilości te mają charakter orientacyjny i mogą się zmieniać w zależności od potrzeb szpitala. Zamawiający dopuszcza zmianę ilości asortymentowych w danej pozycji w zakresie wartości pakietu, na jaki zostanie zawarta umowa. 2. Wykonawca zobowiązuje się dostarczyć przedmiot umowy własnym transportem lub za pośrednictwem profesjonalnej firmy kurierskiej, na swój koszt i ryzyko, do siedziby: 1) Apteka Wielospecjalistycznego Szpitala Powiatowego S.A., ul. Pyskowicka 47-51, 42-612 Tarnowskie Góry lub innej placówki wskazanej przez Zamawiającego - dotyczy pozycji nr 1.1, 1.2 2) do Działu farmacji szpitalnej lub innego wskazanego w zamówieniu miejsca w siedzibie Zamawiającego - dotyczy pozycji nr 1.3, 1.4 - gdzie nastąpi jego rozładunek i wydanie uprawnionym osobom - z tym zastrzeżeniem, że pracownikowi Wykonawcy wykonującemu swoje obowiązki zapewnione zostanie bezpieczeństwo m.in. poprzez unikanie bliskich kontaktów z personelem medycznym oraz z chorymi, w szczególności poprzez zapewnienie i przestrzeganie rozdzielnych dróg dostępu/kontaktów przeznaczonych odrębnie do obsługi pacjentów oraz odrębnie do obsługi dostaw produktów leczniczych przez pracowników hurtowni farmaceutycznych.
