1. Przedmiotem zamówienia jest dostawa produktów farmaceutycznych w asortymencie i ilościach wyszczególnionych w Załączniku nr 2 do IDW - Formularz asortymentowo - ilościowy 2. Oferowany przedmiot zamówienia musi spełniać wymagania Zamawiającego wymienione w Formularzu asortymentowo-ilościowym, być fabrycznie nowy i gotowy do użytkowania zgodnie z przeznaczeniem bez dokonywania dodatkowych zakupów. 3. Wszystkie zaoferowane produkty muszą być dopuszczone do obrotu i używania na terytorium RP zgodnie z polskim prawem oraz prawem Unii Europejskiej. 4. Zamawiający dopuszcza składanie ofert na poszczególne części zamówienia. Niniejsze zamówienie składa się z 15 części (pakietów). Ofertę można złożyć w odniesieniu do jednej lub więcej części. 5. W częściach 1- 12 Zamawiający wymaga zaoferowania produktów leczniczych dopuszczonych do obrotu w Polsce w rozumieniu ustawy Prawo farmaceutyczne z dnia 6 września 2001 roku. Wyjątek stanowi: pozycja 43 części 2, dla której Zamawiający dopuszcza zaoferowanie produktów o innym statusie niż produkt leczniczy, jeżeli spełniają one pozostałe wymagania zawarte w SWZ.
