Description

1. Przedmiotem zamówienia jest sukcesywna dostawa leków na potrzeby Zamawiającego, w tym leków stosowanych w ramach programów lekowych finansowanych przez Narodowy Fundusz Zdrowia. Oferowane produkty lecznicze muszą być dopuszczone do obrotu na terytorium Rzeczypospolitej Polskiej, posiadać aktualne pozwolenia na dopuszczenie do obrotu oraz spełniać wymagania określone w przepisach ustawy - Prawo farmaceutyczne. Dostawy muszą być realizowane zgodnie z zasadami Dobrej Praktyki Dystrybucyjnej (GDP), z zachowaniem wymaganych warunków transportu i przechowywania, w tym łańcucha chłodniczego - w przypadku produktów tego wymagających. Każda dostarczona partia leków musi posiadać termin ważności umożliwiający ich bezpieczne i racjonalne wykorzystanie przez Zamawiającego. Szczegółowy wykaz asortymentu, w tym nazwy, postacie, dawki, ilości oraz wymagania jakościowe, określono w formularzu asortymentowo-cenowym stanowiącym załącznik nr 2 do Specyfikacji Warunków Zamówienia. 2. Zamówienie zostało podzielone na 34 zadania, które - ze względu na zróżnicowany oraz skomplikowany charakter asortymentu - zostały szczegółowo opisane w formularzu asortymentowo-cenowym stanowiącym załącznik nr 2 do Specyfikacji Warunków Zamówienia. 3. Zamawiający dopuszcza możliwość składania ofert częściowych, na jedną lub więcej wybranych części (także na wszystkie części). Wybór oferty najkorzystniejszej nastąpi oddzielnie dla każdej części zamówienia 4. Szczegółowy opis przedmiotu zamówienia zawarty jest w Załącznikach nr 2 do SWZ tj. w formularzu asortymentowo-cenowym (zwanym również OPZ). Ze względu na różnorodność szczegółowy wykaz asortymentu, w tym nazwy, postacie, dawki, ilości oraz inne wymagania określono w formularzu asortymentowo-cenowym stanowiącym załącznik nr 2 do Specyfikacji Warunków Zamówienia.
