Description

Przedmiotem zamówienia jest dostawa leków do obsługi programów lekowych w pakietach nr 1, nr 2, nr 4, nr 6 - nr 7 , płynów do dializoterapii, środków żywieniowych specjalnego przeznaczenia, produktów leczniczych oraz testów ciążowych wraz z testami do wykrywania narkotyków dla SPSZOZ „Zdroje”. Przedmiot zamówienia jest podzielony na 11 części zwanych pakietami. Zamawiający dopuszcza składanie ofert częściowych. Oferta powinna obejmować całość przedmiotu zamówienia w danym pakiecie. Zamawiający przewiduje - w Pakiecie (zadaniu) nr 1 - nr 4, nr 6 - nr 7 - zawarcie umowy ramowej: 1) zawarcie umowy ramowej ze względu na specyfikę przedmiotu zamówienia z jednym wykonawcą; 2) udzielenie zamówienia - zgodnie z art. 313 ust 1 ustawy Pzp - na warunkach zamówienia określonych w umowie ramowej, bez przeprowadzania postępowania o udzielenie zamówienia. Pakiet nr 1- Immunoglobuliny do stosowania w programie lekowym B 17; Pakiet nr 2 - Program leczenia substytucyjnego; Pakiet nr 3 - Żywienie domowe; Pakiet nr 4 - Tofacitinibum; Pakiet nr 5 - Płyny do ciągłej terapii urządzeniem MULTIFILTRATE BASIC; Pakiet nr 6 - Palivizumab PR; Pakiet nr 7 - Ravulizumab; Pakiet nr 9 - ROCURONIUM; Pakiet nr 10 - Wapno sodowane w opakowaniach jednorazowych; Pakiet nr 11 - DERIZOMALTOZA ŻELAZA
