Przedmiotem zamówienia są sukcesywne dostawy materiałów opatrunkowych, kompresów, matryc hemostatycznych, klejów tkankowych oraz siatek przepuklinowych dla Zespołu Opieki Zdrowotnej w Bolesławcu. Parametry opisane w Załączniku nr 2.1-2.5 do SWZ jako wymagane parametry techniczno-użytkowe stanowią szczegółowy opis przedmiotu zamówienia, których niespełnienie spowoduje odrzucenie oferty jako niezgodnej z warunkami zamówienia na podstawie art. 226 ust. 1 pkt 5 ustawy Pzp. Oferowane towary muszą spełniać wszystkie parametry określone w SWZ i załącznikach oraz wymagania jakościowe określone w dokumentacji technicznej producenta na dany wyrób, być fabrycznie nowe (I kat.), nieużywane i wolne od wad oraz posiadać nienaruszone cechy pierwotnego opakowania, z oznaczeniami producenta i numeru katalogowego (jeśli występuje). Zamawiający wymaga towaru wyłącznie profesjonalnego. Każdy Wykonawca może złożyć tylko jedną ofertę na całość zamówienia lub dane zadanie/ zadania w formie pisemnej pod rygorem nieważności. W przypadku złożenia przez Wykonawcę więcej niż jednej oferty na całość zamówienia lub na dane zadanie/zadania wszystkie oferty tego Wykonawcy zostaną odrzucone przez Zamawiającego. Zamówienie składa się z 5 zadań. Zamawiający dopuszcza składanie ofert częściowych. Za ofertę częściową Zamawiający uzna ofertę złożoną na wszystkie pozycje w danym zadaniu.
