Przedmiotem zamówienia jest dostawa kartridżów do zamkniętego systemu biologii molekularnej wraz z dzierżawą aparatu w systemie zamkniętym do diagnostyki mikrobiologicznej metodą PCR (Zadanie Nr 1), serwis aparatu GeneXpert (Zadanie Nr 2). 1.1. Przedmiotem zamówienia jest dostawa kartridżów do zamkniętego systemu biologii molekularnej wraz z dzierżawą aparatu w systemie zamkniętym do diagnostyki mikrobiologicznej metodą PCR (Zadanie Nr 1). 1.2. Specyfikację asortymentowo-ilościowo-cenową przedmiotu zamówienia w przypadku Zadania Nr 1 określa Załącznik Nr 2/1 do SWZ, który Wykonawca zobowiązany jest wypełnić i załączyć do oferty. 1.3. Specyfikację cenową za dzierżawę w przypadku Zadania Nr 1 stanowi Załącznik Nr 2/1a do SWZ, który Wykonawca jest zobowiązany wypełnić i załączyć do oferty. 1.4. Specyfikację techniczną przedmiotu dzierżawy (z wymaganymi i punktowanymi parametrami techniczno-użytkowymi przedmiotu zamówienia) w przypadku Zadania Nr 1 określa Załącznik Nr 2/1b do SWZ, który Wykonawca zobowiązany jest wypełnić i załączyć do oferty.; 1.1. Przedmiotem zamówienia jest serwis aparatu GeneXpert (Zadanie Nr 2). 1.2. Specyfikację określającą rodzaj (typ/model) serwisowanego sprzętu ze wzorem specyfikacji cenowej w przypadku Zadania Nr 2 określa Załącznik Nr 2/2 do SWZ, który Wykonawca zobowiązany jest wypełnić i załączyć do oferty. 1.3. Szczegółowe wymagania dotyczące przedmiotu zamówienia z opisem zakresu usług (czynności) serwisowych w przypadku Zadania Nr 2 określa Załącznik Nr 2/2a do SWZ, który Wykonawca zobowiązany jest wypełnić i załączyć do oferty.
