Description

Przedmiotem zamówienia jest dostawa sprzętu dla Kliniki Neurochirurgii i Neurotraumatologii (w tym depozyt, dzierżawa oraz użyczenie). Przedmiot zamówienia został podzielony na 42 części. Zamawiający żąda złożenia wraz z ofertą przedmiotowych środków dowodowych na potwierdzenie, że oferowane produkty spełniają określone przez Zamawiającego wymagania oraz które pozwolą na jednoznaczną identyfikację zaoferowanego asortymentu, potwierdzając zgodność z opisem przedmiotu zamówienia. Zastosowanie ma art. 105 i 106 ust. 3 ustawy Pzp. WYKONAWCA ZOBOWIĄZANY JEST ZAŁĄCZYĆ DO OFERTY: Dokument poświadczający posiadanie przez producenta normy ISO 13485 lub równoważnej dla oferowanych wyrobów medycznych (w przypadku normy równoważnej należy zamieścić taką informację w ofercie) - nie dotyczy części 38A, 25C. W przypadku gdy Wykonawca nie wskaże normy równoważnej, Zamawiający uzna, że Wykonawca wykazuje dla producenta wyrobu medycznego normę ISO 13485. Część 3 Implant stawowy do kregosłupa lędźwiowego; Część 9 Kaniulowany, samorozszerzający implant międzykolczysty o helikalnej końcówce; Część 14 Zestaw do stabilizacji skoliozy /tytan/; Część 19 Proteza pierścienia włóknistego do dynamicznej stabilizacji krążka międzykręgowego w odcinku lędźwiowym; Część 27 Śruba kaniulowana do stabilizacji złamanego zęba obrotnika.; Część 29 Zestaw implantowy do redukcji złamań trzonów kręgowych; Część 34 Prowadnica, igły i wiertło; Cześć 41 Syntetyczny substytut kostny
