Description

1. Przedmiotem zamówienia są sukcesywne dostawy wyrobów medycznych przeznaczonych do zabiegów elektrofizjologicznych PFA wraz z dzierżawą konsoli PFA i systemu do elektroautonomicznego mapowania serca kompatybilnych z oferowanymi wyrobami. Zamawiający, po zawarciu umowy, wymaga zabezpieczenia przedmiotu umowy poprzez złożenie depozytu wyrobów medycznych. Szczegółowy opis przedmiotu zamówienia stanowi Formularz asortymentowo-cenowy będący Załącznikiem nr 2A do SWZ. 2. Zamawiający wymaga, aby oferowany asortyment spełniał warunki określone w przepisach prawa, w szczególności w ustawie z dnia 7 kwietnia 2022 r. o wyrobach medycznych i w Rozporządzeniu Ministra Zdrowia w sprawie świadczeń gwarantowanych z zakresu leczenia szpitalnego z dnia 22 listopada 2013 r. (Dz.U. z 2023 r. poz. 870 z późn. zm.).
