Description

Przedmiotem zamówienia jest sukcesywna dostawa wyrobów medycznych okulistycznych. Zamówienie podzielone jest na 11 pakietów (części). Umowy w zakresie poszczególnych pakietów zostaną zawarte po zakończeniu niniejszego postępowania na okres 7 miesięcy. Szczegółowy opis przedmiotu zamówienia, opis wymagań zamawiającego w zakresie realizacji dostaw określają: a) Specyfikację dostawy zawiera Załącznik nr 1 do SWZ („Formularz oferty”), a szczegółową charakterystykę stanowi Załącznik nr 1.1 do SWZ („Formularze cenowe”). b) Projektowane postanowienia umowy - Załącznik nr 2 do SWZ.
