Description

Przedmiotem Zamówienia jest sukcesywna dostawa sprzętu medycznego (zwanego dalej również artykułami medycznymi lub asortymentem) dla potrzeb oddziałów Szpitala Powiatu Bytowskiego Sp. z o.o., na podstawie umowy ramowej, zamawianych przez Aptekę szpitalną w asortymencie oraz wymaganiach bezwzględnych określonych w formularzu asortymentowo- cenowym - załącznik nr 1 do SWZ. Załącznik nr 1 do SWZ określa również orientacyjną ilość artykułów medycznych. W okresie obowiązywania Umowy Zamawiający zobowiązany jest do zamówienia dostaw o wartości co najmniej 40% łącznego maksymalnego wynagrodzenia określonego w § 2 ust. 1 Umowy, której wzór stanowiącej załącznik nr 3 do SWZ. Dostawa asortymentu będzie realizowana na podstawie zamówień częściowych na koszt i ryzyko Wykonawcy do Apteki szpitalnej w terminie 3 dni roboczych od chwili złożenia zamówienia.
