Przedmiotem zamówienia są dostawy sukcesywne materiałów zużywalnych do Pracowni Elektrofizjologii Klinicznej Zamawiającego. Opis przedmiotu zamówienia znajduje się w załączniku nr 2 do SWZ (formularz asortymentowo-cenowy). Wymagany termin realizacji dostaw częściowych: 4 dni robocze od złożenia zamówienia. Minimalny termin ważności asortymentu: 9 miesięcy od daty dostawy. Zamawiający zobowiązany będzie do wykorzystania minimum 75% wartości brutto udzielonego zamówienia. Wykorzystanie zależne będzie od liczby pacjentów zgłaszających się do Zamawiającego oraz od umów (kontraktów) z Narodowym Funduszem Zdrowia. Zamawiający nie dopuszcza możliwości składania ofert częściowych Zamawiający nie dopuszcza składania ofert wariantowych. 1.
