Description

1. Przedmiotem zamówienia jest dostawa wyrobów medycznych stosowanych w neurochirurgii dla UCK w asortymencie, szacunkowej ilości oraz wymaganiach określonych w załączniku nr 3 do SWZ. 2. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze pakiety ustalone w załączniku nr 3 do SWZ. Wykonawca może złożyć ofertę na jeden, dwa, trzy lub cztery pakiety. 3. Zamawiający wymaga, aby zaoferowany asortyment spełniał wymagania dotyczące wyrobów medycznych zawarte w ustawie z dnia 7 kwietnia 2022 roku o wyrobach medycznych (Dz. U. z 2024 r. poz. 1620 tj.) oraz w wydanych na podstawie tejże ustawy, aktach wykonawczych. 4. Zaoferowany asortyment musi spełniać wymagania określone w załączniku nr 3 do SWZ. Niespełnienie chociażby jednego z wymagań określonych w załączniku nr 3 do SWZ spowoduje odrzucenie oferty. 5. Dostawa asortymentu niniejszego zamówienia, będzie realizowana na podstawie zamówień częściowych. Maksymalny termin dostawy wynosi 6 dni roboczych od daty złożenia zamówienia przez Zamawiającego. 6. Asortyment, o którym mowa w ust. 1 Wykonawca zobowiązany będzie dostarczyć na własny koszt do Działu Zaopatrzenia Medycznego, ul. Smoluchowskiego 17. Szczegółowy opis przedmiotu zamówienia znajduje się w Załączniku nr 3 do SWZ.
