Description

Przedmiotem zamówienia jest sukcesywne dostarczanie sprzętu jednorazowego użytku w podziale na 27 Pakiety. Zamawiający dopuszcza składanie ofert częściowych w podziale na 27 Pakietów: Pakiety z możliwością składania ofert na poszczególne pozycje -PODZIELNE: 22-27 pozostałe pakiety są niepodzielne bez możliwości składania ofert na poszczególne pozycje. Szczegółowy opis przedmiotu zamówienia określa załącznik nr 9 do SWZ. Dostawa przedmiotu zamówienia do Magazynu Miejskiego Centrum Medycznego im. dr. Karola Jonschera w Łodzi (ul. Milionowa 14, 93-113 Łódź) realizowana będzie na koszt i ryzyko Wykonawcy, w przypadku Pakietów 15, 16, 17, 18, 26, dostawa do Apteki Szpitalnej (ul. Milionowa 14, 93-113 Łódź). Dla Pakietu 3 poz. 9 i 10 Zamawiający wymaga użyczenia monitorów kompatybilnych z zaoferowanym sprzętem jednorazowego użytku na cały okres trwania umowy. Użyczony sprzęt powinien posiadać wszelkie dokumenty niezbędne dla sprzętu medycznego, określone przepisami prawa (instrukcja użytkowania w języku polskim, aktualny paszport techniczny, dokument potwierdzający, że sprzęt jest wyrobem medycznym).
