Description

Przedmiotem zamówienia są dostawy implantów, narzędzi i akcesoriów do zabiegów osteosyntezy. Zamawiający dzieli zamówienie na części, które nazywa pakietami. Pakiet nr 1 - Płytki blokowane Pakiet nr 2 - Gwoździe śródszpikowe Pakiet nr 3 - Osteosynteza kości drobnych Pakiet nr 4 - Osteosynteza płyty dedykowane Pakiet nr 5 - Osteosynteza- koniec bliższy kości udowej i miednica Pakiet nr 6 - Osteosynteza płyty dedykowane- kończyna dolna Pakiet nr 7 - Implanty dedykowane w chirurgii miednicy Pakiet nr 8 - Gwoździe śródszpikowe elastyczne Pakiet nr 9 - Substytut kostny Pakiet nr 10 - Narzędzia dedykowane do usuwania uszkodzonych łączników metalowych Pakiet nr 11 - Implanty do rekonstrukcji uszkodzonych więzadeł i ścięgien Dla pakietu nr 1-8 wymagane jest instrumentarium. Instrumentarium powinno być w kontenerach do sterylizacji w systemie bezobsługowym otwartym. Instrumentarium powinno znajdować się na terenie Szpitala przez cały okres stosowania dostarczonych implantów. Wykonawca utworzy nieodpłatnie na terenie Bloku Operacyjnego magazyn depozytowy z możliwością uzupełnienia zużytych implantów. Dla pakietu nr 1 poz.3-7, pakietu nr 2 poz. 49, pakietu nr 6 poz. 18-23, pakietu nr 7 poz. 1-3 instrumentarium wraz z implantami powinno być dostarczone na Blok Operacyjny w systemie „lotnym” na czas trwania zabiegu operacyjnego. Magazyn depozytowy powinien znajdować się na terenie Szpitala - Blok Operacyjny. Uwaga! Zamawiający przed zawarciem umowy będzie żądał przedłożenia numerów katologowych produktów wraz z wyceną. Dalszy i dokładny opis przedmiotu zamówienia (wielkość, zakres, rodzaj i ilość dostaw) wraz z formularzem asortymentowo - cenowym znajduje się w załączniku nr 1 do Specyfikacji Warunków Zamówienia.
