1. Przedmiot zamówienia stanowią sukcesywne dostawy implantów do zabiegów neurochirurgicznych zgodnie z zapisami w formularzu cenowym stanowiącym - załącznik nr 2 do SWZ. Zamawiający, na czas trwania umowy dostawy, która jest przedmiotem niniejszego postępowania, przewiduje również zawarcie z Wykonawcą umów użyczenia, o treści zgodnej z załącznikiem nr 5a do SWZ - według zapisów zawartych w Załączniku nr 2 do SWZ. Ze względu na ich nieodpłatny charakter nie są one jednak objęte niniejszym postępowaniem o udzielenie zamówienia publicznego. Przedmiotowe zamówienie obejmuje 5 pakietów (części). Przedmiotem zamówienia są sukcesywne dostawy implantów jednorazowych ortopedycznych - zgodnie z zapisami w formularzu cenowym stanowiącym - Załącznik nr 2 do SWZ.
