Description

1. Przedmiotem zamówienia Dostawa implantów dla Chirurgii urazowo-ortopedycznej. Przedmiot zamówienia został podzielony na 4 części/Pakietów, z których każdy stanowi oddzielny przedmiot zamówienia: Pakiet nr 1 - Dostawa gwoździ śródszpikowych Pakiet nr 2 - Dostawa PŁYT LCP Pakiet nr 3 - Dostawa substytutu kości( pasta) Pakiet nr 4 - Dostaw implantów do rekonstrukcji w obrębie stawu kolanowego 2. Szczegółowy opis przedmiotu zamówienia znajduje się w załączniku nr 1a do SWZ. Składane oferty muszą zawierać pełny zakres przedmiotu zamówienia w zakresie danego pakietu. 3. Przedmiot i warunki realizacji niniejszego zamówienia winny być zgodne z ustawą z dnia 7 kwietnia 2022 roku o wyrobach medycznych oraz rozporządzeniem Parlamentu Europejskiego i Rady (UE) 2017/745 z dnia 5 kwietnia 2017 r. w sprawie wyrobów medycznych albo rozporządzeniem Parlamentu Europejskiego i Rady (UE) 2017/746 z dnia 5 kwietnia 2017 r. w sprawie wyrobów medycznych do diagnostyki in vitro (o ile dotyczy) oraz z innymi obowiązującymi przepisami prawymi w tym zakresie, o ile oferowany przedmiot jest wyrobem medycznym. 4. Zamawiający nie przewiduje możliwości udzielenia zamówień, o których mowa w art. 214 ust. 1 pkt 8 ustawy Pzp. Pakiet nr 1 - Dostawa gwoździ śródszpikowych; Pakiet nr 2 - Dostawa PŁYT LCP; Pakiet nr 3 - Dostawa substytutu kości( pasta); Pakiet nr 4 - Dostaw implantów do rekonstrukcji w obrębie stawu kolanowego
