Description

Postępowanie o udzielenie zamówienia publicznego prowadzone jest w trybie przetargu nieograniczonego na podstawie art. 132 i następnych ustawy z dnia 11 września 2019 r. - Prawo zamówień publicznych (Dz. U. z 2024r. poz. 1320 ze zm.) zwanej dalej także „Pzp”. Zamawiający przewiduje zastosowanie tzw. procedury odwróconej, o której mowa w art. 139 ust. 1 ustawy Pzp tj. Zamawiający najpierw dokona badania i oceny ofert, a następnie dokona kwalifikacji podmiotowej Wykonawcy, którego oferta została najwyżej oceniona, w zakresie braku podstaw wykluczenia oraz spełnienia warunków udziału w postępowaniu. Przedmiotem zamówienia są Dostawy produktów leczniczych, wyrobów medycznych oraz substancji do receptury według poniższych części: Zadanie 1 - Zadanie 57. Szczegółowy opis przedmiotu zamówienia ww. części został wykazany i opisany w „Formularzu cenowym” (załącznik nr 2) oraz we Wzorze umowy (załącznik nr 3) stanowiących integralną część Specyfikacji Warunków Zamówienia. W przypadku zaistnienia podstaw do powierzenia przetwarzania danych osobowych Wykonawca zobowiązuje się do podpisania umowy powierzenia przetwarzania danych osobowych (zgodnie z załącznikiem nr 14 do SWZ). W treści ogłoszenia podano szacunkową całkowitą wartość przedmiotu zamówienia z "prawem opcji" - dotyczy wartości całego zamówienia, a także poszczególnych jego części. Przed wszczęciem postępowania nie przeprowadzono wstępnych konsultacji rynkowych. Szczegółowy opis przedmiotu zamówienia ww. części został wykazany i opisany w „Formularzu cenowym” (załącznik nr 2) oraz we Wzorze umowy (załącznik nr 3) stanowiącym integralną część Specyfikacji Warunków Zamówienia. W ogłoszeniu podano szacunkową całkowitą wartość przedmiotu zamówienia z "prawem opcji".
