1. Przedmiotem zamówienia jest zakup, dostawa i rozbudowa systemów informatycznych - Digitalizacja dokumentacji medycznej istotnej z punktu widzenia leczenia i profilaktyki - Rozbudowa systemu do digitalizacji dokumentacji medycznej wraz z integracją do systemu P1 w siedzibie Zamawiającego tj.: WSS Nr 4 w Bytomiu, Al. Legionów 10, 41-902 Bytom. 2. Przedmiot realizowany jest w ramach Projektu KPOD.07.03-IP.10-0419/25 pn. Przyspieszenie procesów transformacji cyfrowej ochrony zdrowia poprzez rozwój usług cyfrowych w Wojewódzkim Szpitalu Specjalistycznym nr 4 w Bytomiu” w ramach inwestycji D1.1.2 „Przyspieszenie procesów transformacji cyfrowej ochrony zdrowia poprzez dalszy rozwój usług cyfrowych w ochronie zdrowia”. 3. Szczegółowy opis przedmiotu zamówienia określa Załącznik Nr 1A - Formularz asortymentowo - cenowy, Załącznik Nr 1B - Opis przedmiotu zamówienia. Informacje w zakresie zasad realizacji dostawy, gwarancji oraz wynagrodzenia zawiera Załącznik Nr 6 do SWZ - Projektowane postanowienia umowy.
