NHS South West London ICB - Long-Term Conditions (LTC) Community Outreach, Expert Patient Programme (EPP) LTC Self-Management And Pentathlon Healthy Lifestyle Programme Services

NHS South West London Integrated Care Board (Croydon Place) commissioned services for Long-Term Conditions community outreach, an Expert Patient Programme for LTC self-management, and a Pentathlon Healthy Lifestyle Programme. The services deliver preventative community health interventions addressing health inequalities in Black, Asian and Minority Ethnic communities, which make up circa 50% of the ICB's core20 communities, particularly in deprived and underserved wards such as Thornton Heath and New Addington. The programme raises awareness of LTC risks, supports healthier lifestyles, early identification and diagnosis, and empowers patients to manage their own conditions. The contract was awarded following a competitive tender under the Scheme of Reservation and Delegation. The winner is Asian Resource Centre of Croydon Limited.
