Description

De inhoud van de Opdracht betreft het uitvoeren van zorgondersteuning in het kader van Beschermd Wonen. Het inkooptraject richt zich op de inkoop van maatwerkvoorzieningen. Beschermd wonen wordt geboden aan inwoners die als gevolg van psychiatrische problemen of een licht verstandelijke beperking, al dan niet in combinatie met psychosociale problematiek, verslaving, delinquentie, armoede, schulden of woonproblemen het niet redden zonder ondersteuning vanuit de gemeente. Het gaat hierbij uitsluitend om zorg in natura (en dus niet om beschermd wonen via een persoonsgebonden budget). - Beschermd Wonen Intramuraal, inclusief dagbesteding; - Beschermd Wonen Intramuraal, inclusief dagbesteding voor Jongvolwassenen 18 - 23 jaar; - Geclusterd Wonen zwaar, inclusief dagbesteding; - Geclusterd Wonen standaard, inclusief dagbesteding; - Beschermd Thuis, inclusief dagbesteding; - Time-out voorziening. De totale waarde van de raamovereenkomst zal over de gehele looptijd nooit meer bedragen dan 140.426.040,30 euro.
