Description

In de basisovereenkomst Wmo-Jeugd2020 worden afspraken tussen gemeente en aanbieder gemaakt over de randvoorwaarden waaraan een aanbieder moet voldoen om in Nissewaard ondersteuning te leveren onder de jeugdwet en/of Wet maatschappelijke ondersteuning vanaf 1 januari 2020. De basisovereenkomst wordt aangevuld met één of meer uitvoeringsovereenkomsten. Alleen partijen die zowel de basisovereenkomst als één of meer uitvoeringsovereenkomsten en de verwerkersovereenkomst hebben ondertekend mogen vanaf 1 januari 2020 in Nissewaard ondersteuning leveren. Alle relevante verslagen en documenten omtrent de Open House procedure Wmo Jeugd 2020 in Nissewaard zijn te vinden op https://www.nissewaard.nl/zorg-en-welzijn/open-house-wmo-jeugd.htm. De getekende basisovereenkomsten, alsmede rechtsgeldig ondertekende UEA (bijlage II), en akkoordverklaring (bijlage III) en andere gevraagde informatie conform de checklist dienen te worden ingediend per e-mail naar: contractbeheer@nissewaard.nl. >€ 750.000,- Jeugdhulp; WMO
