Description

CIG: BCB348C841 Accordo di salute per l’accoglienza dell’utente ID 7229626 presso la struttura “Villa Fabiola” di Fomess SRL. Durata presunta del contratto dal 12/08/2026 al 11/08/2028. Importo base d'asta non superabile € 45.230,63 IVA ESENTE (corrispondente alla sola quota di competenza dell’Ausl di Modena - CSM Carpi per l’accoglienza dell’utente in oggetto, pari all’37,5% della retta giornaliera di € 165,00 IVA ESENTE, moltiplicati per i n. 731 giorni di accoglienza dell’utente), che si intende comprensiva di eventuali prestazioni aggiuntive concordate nel PTRI in base alle tariffe di cui all’Allegato B dell’offerta. E’ prevista l’opzione di ampliamento del contratto per far fronte ad eventuali incrementi tariffari per ulteriori € 13.569,19 IVA ESENTE. Importo complessivo di gara, comprensivo dell’opzione, € 58.799,82 IVA ESENTE. È fatta salva la facoltà di recedere anticipatamente dal contratto a seguito delle valutazioni periodiche effettuate dell’equipe in Unità di Valutazione Multidisciplinare (UVM).
