Description

Gegenstand des Auftrags ist die Beschaffung von Lieferung/Implementierung und Lizenzen zur Installation und Verwendung der Software „Care-Bridge Entlassportal“ (hier: Care Bridge). Care-Bridge ist ein webbasiertes Portal zur Organisation von Versorgungen im Anschluss an die stationäre Krankenhausbehandlung gemäß Rahmenvertrag Entlassmanagement nach § 39 Abs. 1a SGB V zur Nutzung an einem stationären PC, Laptop oder zur mobilen Verwendung auf einem Tablet. Die für das Entlassmanagement relevanten Informationen und Dokumente im Zusammenhang mit stationären und vorstationären Fällen werden über einen KIS-Konnektor automatisch an die Software Care-Bridge übertragen und führen dort zur Anlage eines sogenannten Entlass-Cases. Eine Middleware (Care-Bridge Engine) zwischen KIS und Care-Bridge stellt sicher, dass nur Falldaten an Care-Bridge übermittelt werden, zu denen der Patient seine Einwilligung erteilt hat. Optional kann das Zustimmungsmanagement erweitert werden, um ohne aufwändige Einzelrücksprache mit dem Patienten geeignete Leistungserbringer auszuwählen und kontaktieren zu können. Durch die Beschaffung soll insgesamt sichergestellt werden, dass ein effizienter Entlassprozess mithilfe derer ein volldigitalisierter Prozess in Verbindung mit den Krankenhausinformationssystemen von der Patientenaufnahme bis zur -entlassung abbildbar wird und damit die Patientensicherheit wesentlich verbessert werden kann.
