Rahmenvertrag zur Förderung eines gesundheitsförderlichen Um-gangs für die Lebenswelten Schule und Vereine (gemäß gemäß § 20a SGB V) für die AOK Rheinland-Pfalz/Saarland - Die Gesundheitskasse. Für die gesetzliche Krankenversicherung (GKV) sind der Schutz vor Krankheiten und die Förderung der Gesundheit unverzichtbare Aufgaben, um Lebensqualität und Leistungsfähigkeit ihrer Versicherten unabhängig von Geschlecht und sozialer Stellung langfristig zu erhalten und ihnen ein gesundes Altern zu ermöglichen. Ziel eines Präventionsprogramm zur Förderung eines gesundheitsförderlichen Umgangs ist, gemäße § 20a SG V i.V.m. dem Leitfaden Prävention des GKV-Spitzenverbandes (als Anlage 2b den Vergabeunterlagen beigefügt), die Vermittlung von persönlichen und sozialen Kompetenzen durch Sport. Primäre Zielgruppe sind Kinder und Jugendliche an Grund - und weiterführenden Schulen bzw. im Verein. Die Umsetzung in den Lebenswelten Schule und Verein erfolgt über Projekttage. Bei Bedarf wird zusätzlich eine Schulung für die Lehrkräfte bzw. Übungsleiter angeboten, damit diese die Inhalte in Ihren Stunden integrieren können. Die Maßnahme wird im Rahmen eines Gesundheitsförderungsprozesses an der Schule bzw. im Verein implementiert. Geplanter Umfang der Leistung während der maximalen Vertragslaufzeit von 4 Jahren: 120 ganze Projekttage mit 2 Trainern (Projekttag mit mind. 4 Zeitstunden) 40 halbe Projekttage mit 2 Trainern (Projekttag mit mind. 2 Zeitstunden) 80 x Bereitstellung einer digitalen Plattform pro Institution (Verein oder Schule) Jährlicher Evaluationsbericht pro Tätigkeitsbericht (4x) Optionale Leistung: 32 Schulungen von Multiplikatoren inkl. der Erstellung eines Projekttagebuchs (pro Schulungstermin, mindestens 2 Zeitstunden)
