Description

Předmětem VZ jsou průběžné dodávky speciálního zdravotnického materiálu ve smyslu ustanovení § 2 zákona č. 375/2022 Sb., o zdravotnických prostředcích a diagnostických zdravotnických prostředcích in vitro (dále jen jako „ZZP“). Konkrétně jsou předmětem VZ průběžné dodávky 3D multielektrodových mapovacích a ablačních katetrů včetně požadovaného příslušenství (dále jen „Zdravotnický materiál", „Zdravotnické prostředky“ nebo „ZP“) pro Krajskou nemocnici Liberec, a.s., pracoviště Elektrofyziologie v souladu se ZD po dobu 2 let od dodání a zprovoznění přístroje 3D mapovacího a ablačního systému, jehož bezplatná zápůjčka po dobu 24 měsíců od jeho dodání a zprovoznění je součástí předmětu plnění. Vzor Smlouvy o výpůjčce je přílohou č. 6 ZD.
