Description

Dit lastenboek heeft betrekking op een overheidsopdracht voor de sluiting van een raamovereenkomst voor de aankoop en levering van IVF-geneesmiddelen, bestemd voor het gebruik binnen de dienst URG van het Regionaal Ziekenhuis Heilig Hart Leuven. De opdracht wordt gegund in de vorm van een raamovereenkomst met meerdere percelen, waarbij elk perceel een afzonderlijke categorie of type geneesmiddel omvat. Inschrijvers kunnen zich kandidaat stellen voor één of meerdere percelen, conform de bepalingen van dit lastenboek. De raamovereenkomst wordt afgesloten voor een initiële looptijd van twee (2) jaar, met de mogelijkheid tot twee (2) verlengingen van telkens één (1) jaar, zodat de maximale looptijd van de raamovereenkomst vier (4) jaar bedraagt. De verlengingen worden uitgeoefend onder dezelfde voorwaarden als deze vastgelegd in de oorspronkelijke raamovereenkomst, behoudens andersluidende bepalingen in dit lastenboek. De in dit dossier vermelde hoeveelheden zijn louter geraamd voor de maximale looptijd van vier (4) jaar en dienen uitsluitend om de inschrijvers toe te laten een correcte en vergelijkbare offerte op te maken. Het betreft geschatte hoeveelheden, waaraan geen enkele verbintenis tot afname kan worden ontleend. De effectieve bestellingen gebeuren in functie van de reële noden tijdens de looptijd van de raamovereenkomst. De technische en productspecifieke vereisten waaraan de aangeboden geneesmiddelen moeten voldoen, zijn opgenomen in Hoofdstuk III - Technische bepalingen van dit lastenboek.
