при узгодженні проєкту договору та разом з договором Постачальник зобов’язаний надати Замовнику наступні супроводжувальні документи, посвідчені Постачальником: - оригінал довідки про походження товару, яким виробник або представник, або дилер, або дистриб'ютор, уповноважений виробником, з наданням підтверджуючого документу повноважень (якщо їх відповідні повноваження щодо вчинення правочинів на розповсюдження його продукції поширюються на територію України) гарантує, що товар, який постачається не перебуває в незаконному обігу, країною-походження товару не є Російська Федерація/Республіка Білорусь/Ісламська Республіка Іран. Вищевказана довідка про походження має бути адресована замовнику та містити наступну інформацію: реквізити особи, яка видала довідку, найменування та ідентифікаційний код Постачальника; номер та дата укладення договору; назву предмета закупівлі; ідентифікатор закупівлі; найменування та кількість одиниць товару, що постачається; назва виробника; країна походження товару. Вищевказана довідка вимагається Замовником з метою підтвердження гарантії поставки товару згідно умов договору та коректного відображення виробника, країни походження товару у специфікації згідно вимог чинного законодавства. Поставка однією партією до 31.10.2026.Моментом постачання вважається дата прибуття товару в місце постачання: 49102, м. Дніпро, вул. Ближня, буд.31, склад Комунального некомерційного підприємства «Дніпровський центр первинної медико-санітарної допомоги №6» Дніпровської міської ради. Розвантаження товару представниками Учасника. Товар повинен супроводжуватися документами (копіями реєстраційних посвідчень МОЗ України з додатками про дозвіл застосування предмета закупівлі у медичній практиці або копіями декларації про відповідність з додатками, сертифікатами якості) завіреними Постачальником належним чином
