Description

Predmetom zákazky je dodávka oftalmologík. Predmet zákazky je rozdelený do 4 samostatných ucelených častí. Uchádzač môže predložiť jednu ponuku na jednu ucelenú časť alebo jednu ponuku na viacero ucelených častí alebo jednu ponuku na celý predmet zákazky (časti č. 1 - č. 4). Verejný obstarávateľ si vyhradzuje právo vybrať uchádzača pre každú ucelenú časť zvlášť. Uchádzač v rámci ucelenej časti, na ktorú predkladá ponuku, musí splniť požadovanú špecifikáciu zadefinovanú v rámci jednotlivej ucelenej časti. Bližšie informácie sú uvedené v súťažných podkladoch. Požadované balenie: 1 liekovka s obsahom zodpovedajúcim 50mg liečiva Predpokladané plnenie je 380 liekoviek v registrovaných liekoch s obsahom 50mg liečiva počas zmluvného obdobia 2 roky.; Požadované balenie: 1 implantát s obsahom 700 mcg liečiva Predpokladané plnenie je 28 implantátov s obsahom 700 mcg liečiva v registrovaných liekoch so spôsobom úhrady „A“ počas zmluvného obdobia 2 roky.; Požadované balenie: 1 injekčná striekačka s objemom 0,24ml a obsahom 28,8 mg liečiva Predpokladané plnenie je 64 injekčných striekačiek s obsahom 28,8 mg liečiva v registrovaných liekoch kategorizovaných so spôsobom úhrady „A“ počas zmluvného obdobia 2 roky.; Požadované balenie: 1 injekčná striekačka s objemom 0,23 ml a obsahom 2,3 mg liečiva Predpokladané plnenie je 44 injekčných striekačiek s obsahom 2,3 mg liečiva v registrovaných biosimilárnych liekoch kategorizovaných počas zmluvného obdobia 2 roky.
