The buyer, the Kirovohrad Regional Hospital municipal enterprise, is purchasing frozen fish, specifically hake. Delivery is to the hospital's food warehouse at 2/5 Universytetskyi Avenue, Kropyvnytskyi, postal code 25030, with unloading included. In line with point 57 of Resolution No. 822 of 14 September 2020, the winning bidder must, within two working days, provide a scanned copy of an original document confirming the goods' country of origin is not the Russian Federation, the Republic of Belarus or the Islamic Republic of Iran. The document, obtained from the manufacturer or its authorised representative, dealer or distributor, must cite the procurement identifier, the supplier's full name, the subject and origin country, and be addressed to the buyer. These certificates form an integral part of the contract.
