Szczegółowy opis przedmiotu zamówienia zawiera: opis produktu, wymagana postać, dawka, pojemność/objętość, ilość w op., jednostka miary, ilość op. określa FORMULARZ ASORTYMENTOWO-CENOWY, stanowiący Załącznik nr 1 do SWZ. Szczegółowe warunki i zasady dotyczące realizacji przedmiotu zamówienia uregulowane są we Wzorze Umowy załącznik nr 4 do SWZ „Warunki umowne”.
