Leki w zakresie zadań nr 1-15 muszą być zarejestrowane w programach lekowych: Zadanie nr 1 - 4 Program lekowy B.157, Zadanie nr 5 -15 - Program Lekowy B.29. Wykonawca oświadcza, że zaoferowane produkty lecznicze w zakresie przedmiotu zamówienia są zarejestrowane ze wskazaniem do stosowania w programie lekowym, zgodnie z aktualnym obwieszczeniem Ministra Zdrowia. Szczegółowy opis przedmiotu zamówienia zawarty został w dokumentacji zamówienia - załączniki numer 2 do SWZ.
