Description
1. Przedmiotem zamówienia są: PRODUKTY LECZNICZE, KONTRASTY dla Wojewódzkiego Szpitala Specjalistycznego im. NMP w Częstochowie. 2. Opis przedmiotu zamówienia został zawarty w formularzach asortymentowo-cenowych, stanowiących Załącznik Nr 2 do SWZ. Pozostałe warunki dotyczące realizacji zamówienia zostały określone w Załączniku Nr 6 do SWZ, stanowiącym projektowane postanowienia umowy. 3. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części ustalone w Załączniku Nr 2 do SWZ. Wykonawca może złożyć ofertę na dowolne części lub na cały przedmiot zamówienia. 4.Przedmiot zamówienia podzielony jest na 12 części, tj.: Części nr 1 - 2 Leki programowe Części nr 3 - 7 Leki podstawowe Części nr 8 - 10 Antybiotyki Części nr 11 - 12 Kontrasty 5. Kod CPV: 33600000-6 (dla części 1-10) 33696000-5 (dla części 11-12)
Przedmiot zamówienia został szczegółowo opisany w ilościach i asortymencie w formularzu asortymentowo-cenowym, stanowiącym Załącznik nr 2 do SWZ.
