Description

1. Przedmiot zamówienia: LEKI II - szczegółowo opisane w załączniku nr 2 do niniejszej SWZ - poszczególne pakiety. 2. Szczegółowy opis przedmiotu zamówienia, opis wymagań zamawiającego w zakresie realizacji i odbioru określają: - załączniki numer 2, 3 do SWZ. 3. Wymagania wyszczególnione w chociażby w jednym dokumencie stanowiącym załączniki do SWZ będą obowiązujące dla Wykonawcy, jakby były zawarte w całej dokumentacji. 4. Podane w formularzach asortymentowo - cenowych ilości (określają zamówienie podstawowe - bez uwzględnienia opcji). Ostateczna wartość przedmiotu umowy zostanie określona na podstawie faktycznej ilości zamówienia, wynikającej z faktycznego zapotrzebowania Zamawiającego, pomnożonej przez cenę jednostkową z zastrzeżeniem, że minimalny poziom realizacji Umowy nie będzie mniejszy niż 70% maksymalnej wartości zamówienia podstawowego, bez uwzględnienia wartości zamówienia objętego prawem opcji i Wykonawcy nie służą żadne roszczenia z tytułu niezrealizowania Umowy powyżej tego poziomu. W przypadku niewykorzystania wartości umowy, Zamawiający może dokonywać zmian (przesunięć) zamawianych ilości pozycji w poszczególnych pozycjach asortymentowych w poszczególnych pakietach. 5. Opis przedmiotu zamówienia należy odczytywać wraz z ewentualnymi zmianami treści niniejszej SWZ, będącymi np. wynikiem udzielonych odpowiedzi na zapytania wykonawców. Szczegółowy opis przedmiotu zamówienia zawarty został w dokumentacji zamówienia - załącznik numer 2 - poszczególne pakiety.
