2.1. Przedmiotem niniejszego zamówienia jest zakup i sukcesywna dostawa leków w programach lekowych oraz innych leków dla Zespołu Szpitali Miejskich w Chorzowie w ilości i opisie określonej w specyfikacji asortymentowo-cenowej stanowiącej załącznik nr 2 do niniejszej SWZ. Przedmiot zamówienia obejmuje 57 Pakiety. Pakiet nr 41 program lekowy B.29.(5) (1 pozycja asortymentowa); Pakiet nr 42 program lekowy B.29.(6) (1 pozycja asortymentowa); Pakiet nr 43 program lekowy B.154.FM.(1 pozycja asortymentowa); Pakiet nr 44 - LEKI STOSOWANE W CHEMIOTERAPII (18 pozycji asortymentowych); Pakiet nr 45 - TRETINOIN ( 1 pozycja asortymentowa); Pakiet nr 46 - CARBOCISTEINE (1 pozycja asortymentowa); Pakiet nr 47 - MIDAZOLAM (3 pozycje asortymentowe); Pakiet nr 48 - AZACITIDINE (1 pozycja asortymentowa)
