5.1. Przedmiotem zamówienia jest sukcesywna dostawa leków, obejmujących cytostatyki, do apteki szpitalnej - zgodnie z nazwą i ilością określoną w zadaniu cenowych nr 1. Przedmiot zamówienia został podzielony na pozycje. Zamawiający dopuszcza składanie ofert częściowych zgodnie z poniższym opisem: Zadanie - leki cytostatyczne: oferta może dotyczyć całego zadania lub poszczególnych pozycji lub pozycji połączonych zgodnie z opisem 1; 2; 3; 4; 5; 6 ;(7+8) Przedmiotem zamówienia jest sukcesywna dostawa leków, obejmujących leki cytostatyczne: Zadanie nr 1 poz. nr 1- Cisplatinum 1mg/ml - oferta powinna dotyczyć całego zadania Zadanie nr 1 poz. nr 2 - Docetaxelum 160 mg/8ml- oferta powinna dotyczyć całego zadania; Zadanie nr 1 poz. nr 3- Gemcitabinum 2000mg- oferta powinna dotyczyć całego zadania; Zadanie nr 1 poz. nr 4 - Topotecanum 4mg/4 ml - oferta powinna dotyczyć całego zadania; Zadanie nr 1 poz. nr 5- Pemetrexedum - oferta powinna dotyczyć całego zadania; Zadanie nr 1 poz. nr 6- Paclitaxelum 6mg/ml 50 ml (300mg) - oferta powinna dotyczyć całego zadania; Zadanie nr 1 - poz. 7 Vinorelbinum kaps 20 mg poz. 8 Vinorelbinum kaps. 30 mg- oferta powinna dotyczyć łącznie poz. nr 7 i poz. nr 8 bez rozbicia na poszczególne pozycje, zadanie łączone; Cisplatinum 1mg/ml konc.do.sporz.r-u do inf fiol.100mg/100ml; Docetaxelum 160mg/8ml vel 160mg/16ml konc.do.sporz.r-u do.inf; Gemcitabinum 2000mg konc do sporządz.r-u do inf; Topotecanum konc do sporządz.r-u do inf. 4mg/4ml fiol.; Pemetrexedum koncentrat do sporządzania r-u do infuzji 25mg/ml fiol 1000mg/40ml; Paclitaxelum konc do sporządz.r-u do inf. 6mg/ml 50ml(300mg); Vinorelbinum kaps.20mg Vinorelbinum kaps.30mg
