Description

Przedmiotem zamówienia jest sukcesywna dostawa leków, obejmujących programy lekowe, do apteki szpitalnej - zgodnie z nazwą i ilością określoną w zadaniach cenowych. Przedmiot zamówienia został podzielony na zadania. Zamawiający dopuszcza składanie ofert częściowych zgodnie z poniższym opisem: Zadanie nr 1 -nivolumab-program lekowy oferta powinna dotyczyć całego zadania; Zadanie nr 2 - ipilimumab-program lekowy- oferta powinna dotyczyć całego zadania; Zadanie nr 3 - atezolizumab-program lekowy- oferta powinna dotyczyć całego zadania; Zadanie nr 4 - pembrolizumab programy lekowy - oferta powinna dotyczyć całego zadania; Zadanie nr 5 - durvalumabum program lekowy - oferta powinna dotyczyć całego zadania; Zadanie nr 6 - MEPOLIZUMAB program lekowy leczenie cieżkiej astmy alergicznej IgE zależnej oraz ciężkiej astmy eozynofilowej- oferta powinna dotyczyć całego zadania; Zadanie nr 7 - OMALIZUMAB program lekowy leczenie ciężkiej astmy alergicznej IgE zależnej- oferta powinna dotyczyć całego zadania bez rozbicia na poszczególne pozycje; Nivolumabum 10mg/ml 40mg/4ml konc.d.sp.roz.d/inf fiol Nivolumabum 10mg/ml 100mg/10ml konc.d.sp.roz.d/inf fiol; Ipilimumab 5mg/ml 50mg/10ml konc.d.sp.roz.d/inf fiol; Atezolizumabum 1200mg konc.d.sp.roz.d/inf 20ml fiolka; Pembrolizumabum 25mg/ml fiolka 4ml konc.d.sp.roz.d/inf; Durvalumabum konc.do sporz,inf 500mg/10ml; MEPOLIZUMAB 100mg; OMALIZUMAB 75mg OMALIZUMAB 150mg
